The Child Health and Disability Prevention (CHDP) program provides a free comprehensive health exam for the early detection and prevention of disease and disabilities in children and youth. Health Care Program for Children in Foster Care (HCPCFC) is funded through CHDP and also provides comprehensive health care, and documentation for youth placed in foster care system. Applications are completed at the office or clinic of an enrolled CHDP provider.
Preventive health exams can help children stay healthily, find problems early, and answer many questions. Through the CHDP program, eligible children and youth receive periodic preventive health exams. Children with suspected problems are then referred for diagnosis and treatment. Many health problems can be prevented or corrected or the severity reduced, by early detection and prompt diagnosis and treatment.
Who provides CHDP services?
Community clinics and private physicians provide CHDP heath assessments at no charge for income-eligible families in Marin.
Is my child eligible for CHDP services?
If your child receives Medi-Cal health benefits, they are eligible for the CHDP program. Children whose families meet the program's income guidelines are also eligible.
What does CHDP offer?
Care Coordination: Local CHDP & Foster Care Public Health Nurses (PHN) will assist families in obtaining diagnostic and treatment services in medical, dental and mental health problems identified during the health assessment. Providers may call the local CHDP Public Health Nurse and request assistance for their CHDP patients.
Referrals: Parents can connect with a Certified Application Assistor who will help determine which insurance program is best for you and your children.
