HAVEN: Homeless Outreach Program

The HAVEN Team provides a comprehensive, integrated continuum of services to individuals who are seriously and persistently mentally ill and who are homeless. All services are guided by the principles of recovery and consumer empowerment. The HAVEN Team has served 160 individuals in Marin County since the program's launch in December of 2000, with additional outreach to over 800 homeless individuals in Marin County. Services (including outreach, case management, psychiatric and medical treatment, and employment support services) are designed to reduce symptoms that impair clients' ability to live independently, work, maintain community supports, care for their children, remain healthy, and avoid psychiatric hospitalizations and crime. Its goal is to help enrollees achieve their highest level of wellness and independence within the community. The frequent contacts and service coordination provided by HAVEN's multidisciplinary team have empowered clients to make great strides in their recovery: - 22% of clients were employed as of December 2005 - 10% of clients were in school as of December 2005 - 51% reduction in the number of days spent in a psychiatric hospital in 2005. This annualized reduction of 287 days represents a potential savings to the county of $179,375 for that year. - 96% of participants had obtained and maintained housing as of December 2005 - 49% overall reduction in the number of days incarcerated since enrollment, over a 5 year-period Services HAVEN's multidisciplinary staff works within a strength-based model, confident that program participants are most successful when they set their own goals and actively engage in their mental health recovery. An intensive case management team works with enrolled members to access the services, resources and supports needed to gain and maintain recovery in the community. Three mental health professionals partner with clients to set and achieve short and long-term employment, educational, medical, life skills, social, housing, and recovery goals. Five trained peer providers provide individual and group peer support services and WRAP (wellness recovery action plan) services. A psychiatrist provides mental health treatment, meeting regularly with clients at both the program's Greenbrae office and the peer-run Enterprise Resource Center in San Rafael. A mental health nurse practitioner is on the HAVEN Team because program enrollees often present with a range of health problems, many of which are quite severe. This mental health nurse practitioner provides medical case management as well physical and mental health care to enrollees requiring those services. Active engagement in the community is seen as a common goal for clients. A part-time employment specialist works with the intensive case management team to assess, identify and support employment options for persons with severe disabilities.
