Confidential Morbidity Reports (CMR)

The California Reportable Disease Information Exchange (CalREDIE) is a computer application that the California Department of Public Health (CDPH) has implemented for web-based disease reporting and surveillance. Specified diseases and conditions are mandated by State laws and regulations to be reported by healthcare providers and laboratories to local public health authorities.

 

Confidential Morbidity Reports (CMR) are now being completed and submitted online through CalREDIE Provider Portal to Marin County's Communicable Disease Prevention and Control Unit (CDPC).

Disease Reporting and Investigation:

The Communicable Disease Prevention and Control Unit (CDPC) is responsible for the investigation of communicable diseases and surveillance of reportable diseases.

Tuberculosis Reporting:

Health care providers must complete a confidential online report for all suspect and active tuberculosis cases among Marin County residents. Please do not wait for laboratory confirmation before reporting.  For more information visit our TB page for Health Care Providers.
