Effective January 24, 2018, Marin HHS Specialty Clinics closed permanently. On February 1, 2018, Marin Community Clinics (MCC) will begin offering primary and specialty care (including infectious disease services) at 3260 Kerner Blvd.  The medical records of patients who are continuing their care with MCC have been transferred to MCC.  If you chose another provider, you may request  a copy or written summary of your health record be transferred to you or another practice of your choice by calling (415) 473-4163 or via fax at (415) 473-6002.

How long should patient records be retained?

10 years after the last visit or significant contact (a recorded event in the patient’s chart)
21 years after treating a pregnant woman
21 years after delivery or treatment of an infant
6 years after death of a patient
Medicare/Medicaid requires original source documentation and medical records for claims to be retained for 6 years, after the bill is paid

Where and how should records be maintained?

Records should be stored to allow for lawful access and in a place that maintains confidentiality
The responsible person, corporation, or legal entity acting in the capacity of custodian of the records must comply with and be subject to federal and/or state confidentiality laws and regulations
Medicare/Medicaid requires patient records to be accessible if records are stored in a location other than the business address on file with those programs. Post office boxes and drop boxes are not acceptable addresses for the storage of patient records.

Disposing of records:

To preserve confidentiality, all records, wherever possible, should be destroyed after the retention period has passed.

Unless otherwise requested, all medical records created by Marin HHS will be archived. If you need a copy of your medical records after the clinic closure, please contact us by mail, phone, fax, or email.
